Subscriber Registration Page

Instructions to self-enroll as a subscriber:

Select "Open In Fullscreen" in the form BELOW.
Leave "Company Name" blank (unless your employer is paying for your subscription).
IF you would like to pre-fill some of your medical history, select "Add more details" at the bottom of the page.
Enter as little or as much as you'd like--it's lengthy, so don't feel obligated to fill it all.
Click "Next" to skip through any sections you don't know or don't apply.
Enter the payment card (Credit or Debit Card or HSA or Flex Card) you will use for your subscription.
You may add family members at the end of the process without having to repeat the address and card information.
After completing the registration form, check your email to sign your consent forms.
Call us at 210-521-6328 to finalize your enrollment and schedule your initial appointment or anytime for problems or questions.
NOTE: Subscription fees start on the day of registration. While you may cancel your subscription at anytime (with the button at the bottom of the page), all fees are non-refundable. Only register and subscribe when you are ready to be our patient. Call us if you have questions or wish to meet to the doctor first. We'll be happy to introduce ourselves and answer questions about our services.

Your subscription (in good standing) and your provider-patient relationship will expire in 30 days.
