As a direct primary care practice, we offer attentive and personalized care to a select panel of patients with increased access and communication.  We offer the COVID19 Vaccine to patients with an established relationship for appropriate screening, treatment and monitoring, in addition to our COVID-safe, convenient primary care.  If you are not looking to subscribe as our patient, please visit www.vaccinefinder.org for other sites that may help you on a vaccine-only basis. 

To make complete use of the vaccine multi-dose vials we offer vaccination BY APPOINTMENT ONLY.  For your safety and convenience, we provide Drive-In Vaccinations at our Stone Oak office.

Drive-In Vaccinations

Before:

If you are a new patient, click on the button to subscribe to our practice.  We will process your registration within 24 hours and schedule your appointment time for the vaccine.  We will text you directions and instructions for your appointment.

During:

Remain in the comfort of your vehicle for a pre-vaccine screening, for information from the CDC, and the vaccine administration.  We'll monitor you for 15 minutes (or longer if warranted by your health).  

After:
We'll provide you the CDC card documenting your vaccine and your return date for the second dose in 24-42 days.  We'll register your vaccination with the state immunization registry to be accessed by other health care providers and hospitals.  As our patient, we remain available to help you with adverse reactions or other primary care needs.

Notes:

- We are an Internal Medicine practice specializing in the care of adults. For your safety and optimal care, we are only providing the vaccine to new patients over the age of 30 and that are not pregnant.  

- All fees are NON-REFUNDABLE.  If you book an appointment elsewhere for the vaccine, we continue available as your primary care home until you terminate your subscription and relationship with us.
